OVERVIEW
BPC-157 Research Overview
SS-31, also known as Elamipretide, is a synthetic peptide designed to interact with mitochondria, the tiny structures inside our cells responsible for producing most of the energy our bodies need. It has attracted scientific interest because mitochondria play an essential role in keeping cells functioning properly, particularly in energy-demanding tissues such as the heart, muscles, and brain. SS-31 has therefore become an important subject of research into cellular energy, mitochondrial function, muscle performance, and conditions associated with impaired energy production.
Research has examined how SS-31 interacts with cardiolipin, a special type of fat found within the inner membrane of mitochondria. This membrane contains much of the machinery cells use to produce energy. By interacting with cardiolipin, SS-31 may help support the structure and function of this energy-producing system. Researchers have also investigated whether these interactions influence oxidative stress, a process in which highly reactive molecules can damage cells when the body's protective systems cannot keep them under control.
SS-31 has progressed through laboratory research, animal studies, and human clinical trials investigating conditions associated with mitochondrial dysfunction. Studies have examined outcomes such as muscle strength, exercise performance, fatigue, and heart function. Research involving people with Barth syndrome, a rare inherited condition affecting mitochondrial function, has been particularly important in understanding the compound's potential medical applications.
Although SS-31 has a substantial clinical research history, its findings should be interpreted within the specific conditions and populations studied. Improvements in mitochondrial measurements do not automatically establish broader benefits for healthy aging, athletic performance, or general energy levels. Continued research is important for understanding its long-term effects and whether findings from specialized patient populations apply to other conditions.

MECHANISM OF ACTION
EVIDENCE AT A GLANCE
Elamipretide has been investigated in laboratory studies and clinical trials involving mitochondrial dysfunction, including research in Barth syndrome and primary mitochondrial myopathy. Clinical findings have varied across conditions, and not all trials met their primary endpoints. In 2025, the FDA granted accelerated approval for a specific Barth syndrome indication, while further research remains important for confirming clinical benefit and evaluating other potential applications.

Proposed Mechanism
SS-31, also known as elamipretide, is a small peptide that targets mitochondria, the structures responsible for producing much of the energy used by cells. It is associated with cardiolipin, a specialized fat molecule concentrated in the inner mitochondrial membrane.
Cardiolipin helps maintain the membrane's structure and supports the protein complexes that generate cellular energy. Elamipretide binds to cardiolipin and has been studied for its effects on mitochondrial organization and function, particularly when these systems are disrupted.
This mechanism has led to research in disorders involving mitochondrial dysfunction. An elamipretide medicine received FDA accelerated approval for a specific rare disease, but that regulatory finding should not be generalized to unrelated conditions or research formulations.
